Local Chapter Membership is from January 1st through December 31st each year and the dates will not coincide with your National H.O.G. Membership. Your National Membership must be current before your 2010 Local Chapter Membership can be activated.

Each Member and Prospective Member must complete and sign the forms each year; couples cannot sign-off together on one form. The actual form cannot be changed or altered from its original format. The information requested on these forms cannot be typewritten; all information must be handwritten or printed with an original signature. Copies will not be accepted.
